महोदय,

मेरा नाम रघुनाथ पिता का नाम मुलुआ है मैं ग्राम जल्ला, पोस्ट झलोखर, थाना सुमेरपुर,जिला हमीरपुर का निवासी हूं, मेरा जन्म सन् 1950 में हुआ था। मैंने सन 2012 में वृद्धा पेंशन का का फॉर्म भरा था जिसके बाद से मुझे वृद्धा पेंशन मिलना चालू हो गई थी पिछले 9 सालो से मुझे पेंशन मिलती आ रही है, पर पिछले एक साल से मेरी पेंशन आना बन्द हो गई है, हमने बैंक में पूछा तो पता चला कि हमारी पेंशन समाज कल्याण विभाग द्वारा बंद कर दी है, महोदय यह पेंशन ही मेरी वृद्धा अवस्था में खाने पीने का सहारा है। पेंशन न मिलने से अब मेरी स्थति बहुत खराब हो गई है मेरे घर में खाने के लिए राशन नहीं है न ही राशन लेने के लिए पैसे है।

अत: आपसे विनम्र निवेदन है कि मुझ गरीब वृद्ध की पेंशन फिर से चालू करने की कृपा करे तो आपकी महान दया होगी।

मैंने इस पत्र के साथ अपना आधार कार्ड, वोटर कार्ड,ग्राम प्रधान द्वारा जारी किया गया निवास प्रमाण पत्र भी लगा दिया है।

My name is Raghunath Father's name is Mulua, I am a resident of Village Jalla, Post Jhalokhar, Thana Sumerpur, District Hamirpur, I was born in the year 1950. I had filled the old age pension form in the year 2012, after which I started getting old age pension, I have been getting pension for the last 9 years, but my pension has stopped coming from the last one year, we asked in the bank So it came to know that our pension has been stopped by the Social Welfare Department, Sir, this pension is the only support for eating and drinking in my old age. Now my condition has become very bad due to non-receipt of pension, there is no ration in my house to eat nor money to take ration.

Therefore, it is a humble request to you that if you please revive the pension of my poor old man, then you will have great mercy.

I have also attached my Aadhar card, Voter card, Residence certificate issued by the village head along with this letter.
